From reading the waring message we can understand that you have violated the fiver TOS, but they have not mentioned as how you have violated.

You carefully read and understand the Fiver TOS, then you can understand how you have violated.

I think it should have said what they think you did wrong. It does say at the end to contact them if you have any questions or concerns which you have. I don’t know whether it would be a good idea to ask them again in the same ticket if they haven’t responded and it’s been a long time or whether they might not like that. Have you done anything that might be against the Fiverr terms of service or anything that they might think is “tricking” their system? eg. auto-refresh add-ons or maybe asking to contact outside of Fiverr or asking for a high rating or some form of review manipulation or anything else that Fiverr might not like?
