Has anyone had any complaints with the W-9 form

I have a question about the Fiverr W-9 form.
I want to post gigs there and for my last requirement, I was wondering if anyone had any trouble with filling out this form there? Some involvement with exchanging information or whatnot? I’m a little worried. I don’t want my information to be construed or tampered with.
