Fiverr’s Choice is a badge that’s manually given to certain gigs on Fiverr. The Fiverr staff (or some team that works there) handpicks sellers’ gigs based on their performance and sales.

Since we’re all mostly buyers and sellers on the Forum, none of us know the exact algorithm that Fiverr uses pick gigs to reward those badges to.
