Every time I come to Fiverr these days, I get irritated by the grammar mistake on the sign in form. It is a small thing and I should have liked to contact the editor directly, but cannot find a way.

As it stands at the moment, you switch from singular in the first half to plural in the second.

You say ‘Give thy name and ye shall pass.’ Correctly, this would be 'Give thy name and thou shalt pass.'

Perhaps someone could pass this information to the appropriate person.
